Tailbone Pain Relief in Chula Vista & San Diego

Sitting is the problem. Driving is the problem. Getting up off a hard chair makes you plant a hand on the table and lift carefully, like someone much older than you are.

Tailbone pain — coccyx pain, coccydynia, call it what you like — is one of the most treatable problems we see and one of the most often shrugged at. Hands-on physical therapy and regenerative tools, one therapist, one hour, all external.

How it usually starts

You landed on it

Ice, a wet step, the bottom stair, a fall off a bike. It settled down over a few weeks and then it stopped settling down, and that was eight months ago.

You had a baby

The coccyx moves during delivery. Sometimes it does not come all the way back, and sometimes the tissue around it stays angry long after everything else has healed.

You sit for a living

No fall, no injury, nothing to point at. Just a desk, a commute and a chair, and a tailbone that started complaining and never stopped.

Nobody knows

A good share of tailbone pain has no clean origin story. That does not mean there is nothing to find — it means nobody has looked properly yet.

Why it drags on for months

The tailbone is not a dead end at the bottom of your spine. It is an anchor point. The pelvic floor attaches to it. So do ligaments running up into the sacrum and down into the glutes.

When one of those attachments is irritated, everything around it tightens up to protect it — and then the tightening becomes the problem in its own right. The coccyx stops moving the small amount it is supposed to move when you sit down. Every time you sit, you load a joint that has lost its give.

Meanwhile the standard advice is a donut cushion and time. The cushion helps you tolerate the day, which is worth something. It does nothing about why the joint stopped moving, which is why six months later you are still carrying it from room to room.

An X-ray that comes back clean does not rule any of this out. It was never going to show it.

What we actually work on

Everything below is done externally, by a licensed Doctor of Physical Therapy, one-on-one.

The tissue around the coccyx

Glutes, the ligaments running into the sacrum, the deep hip rotators. This is where most of the restriction sits and it is all reachable from the outside.

How the pelvis and low back move

A tailbone rarely hurts on its own. If the joints above it have stopped sharing the load, the bottom of the chain takes all of it.

Breathing and the pelvic floor

The pelvic floor attaches directly to the coccyx. If it is gripping, the coccyx is being pulled on all day. Releasing that is often the fastest win in the whole plan.

How you sit and how you stand up

The position, the chair, the car seat, the way you lower yourself into it. Changing this buys relief in days while the rest of the work catches up.

The equipment, and what it is for

This is the kind of problem the regenerative tools were built for — a small, deep, stubborn area of tissue that has stopped responding to time. Most physical therapy clinics in San Diego do not have them.

Focused shockwave

Aimed deep and precisely at one small target. For a stubborn tailbone this is usually the first choice, because the area we need to reach is small and not near the surface.

Radial pressure wave

A handheld applicator working across a broader area — the glutes, the sacrum, the muscle wall around it. Used to improve blood flow and soften tissue that has gone stiff. An FDA cleared radial pressure wave/EPAT device for treating acute and chronic musculoskeletal pain.

EMTT

A magnetic field therapy. No contact and nothing to feel. Used alongside the other two to help tissue respond.

What the first few weeks look like

1. We find out what is driving it

A full assessment with a Doctor of Physical Therapy. Not fifteen minutes. We look at the coccyx, the pelvis, the hips, the low back and how you sit, because the painful spot and the cause are often not the same place.

2. We treat it

Hands-on work, the equipment where it earns its place, and a short set of things to do at home that are specific to you.

3. We make it stick

We keep going until you can sit through a meeting, a flight or a long drive without thinking about it, then we show you how to hold onto it.

Most people notice something inside the first three or four weeks. Tailbone pain that has been there for years takes longer than tailbone pain from a fall last month, and we will tell you which one you have after the assessment.

Being honest about fit

This is probably a fit if:

  • Sitting is the thing that sets it off and it has been going on for more than a few weeks
  • You have been told to use a cushion and wait, and the waiting is not working
  • You would rather try something conservative before injections or surgery
  • You are willing to do a bit of work between visits

This is probably not a fit if:

  • You are looking for a single appointment that fixes it
  • You have had a significant fall and have not been imaged, or you have bowel or bladder changes alongside the pain — see a physician first and we will say so
  • You want internal pelvic treatment. We do not perform internal exams or internal work. Everything we do is external, and if internal work is what you need we will point you to someone who does it

Questions we get asked

Is tailbone pain treated internally?

It can be, in some clinics. We do not do that. We do not perform internal exams or internal treatment — everything we do here is external, and a great deal is reachable that way.

Mine started after childbirth. Is that different?

The approach is the same and the surrounding picture usually is not. We will look at the whole postpartum picture rather than just the tailbone — see our women’s health and pelvic care page.

Does the shockwave hurt?

Most people describe it as a strong tapping. We set the intensity with you and we turn it down the moment you say so.

My X-ray was normal.

That is the usual result and it does not rule anything out. Most of what causes lasting tailbone pain — ligament irritation, lost joint mobility, muscle guarding — was never going to show on an X-ray.

Should I keep using the donut cushion?

Use it if it gets you through the day. Just do not mistake it for treatment — it is managing the symptom, not the cause.
